Your paperwork, all in one place.
Download the forms that apply to you, fill them in, and save them. Then follow the steps below to book your first session and upload everything through the client portal.

Book your first appointment
- 1Open the client portal and click I'm a new client.
- 2Choose Psychiatric Diagnostic Evaluation.
- 3Select Office, or Video Office for a virtual session.
- 4Choose an available day and time.
- 5Fill out the pre-screening.
- 6Fill out your contact information.
Sign back in
- 7Go back to the portal by clicking Soulful Sage Counseling at the top.
- 8Choose I'm an existing client.
- 9Enter the email you used during new client setup and send yourself a link.
- 10Follow the link sent to your email.
Finish your paperwork
- 11Click the Documents tab at the top and fill out the GAD-7 and PHQ-9.
- 12Use the My Upload section to upload your filled-out PDF forms.
Tip for filling out PDFs: open them in the free Adobe Acrobat Reader, or in your computer's browser, then save before uploading. Some phone browsers don't keep your answers, so on a phone use the Acrobat Reader app.

Everyone starting therapy completes these five, whether you're coming on your own or with a partner.
Client Information Form
2 pagesYour contact details, emergency contact, and how you'd like to be reached. It's used for scheduling, billing, and emergencies, and it stays confidential in your client record.
Coming as a couple? This one form has a section for each partner.
Notice of Services
2 pagesWhat Soulful Sage offers, and what it doesn't, so you can be sure it's the right fit for your needs. Sign to confirm you've read it.
Notice of Privacy Practices
2 pagesHow your health information is protected under HIPAA, when it can be shared, and your rights to your records. Sign to confirm you received it.
Payment & Card Authorization
2 pagesAllows a payment method to be kept on file for session fees and late cancellations.
No card numbers go on this form. Your card is entered securely through the client portal.
AI Tools Consent
2 pagesExplains how AI tools are, and are not, used in your care, and lets you choose whether your sessions may be transcribed to help with note-taking.
Either answer is fine. Saying no will not affect the quality of your care.

Pick the agreement that matches the therapy you're starting. Both cover confidentiality, telehealth, cancellations, fees, and your rights as a client.
Individual Therapy Consent
3 pagesFill this out if you're coming to therapy on your own.
Couples Therapy Consent
4 pagesFill this out if you're coming in with a partner. In couples work the relationship is the client, and this agreement explains the no-secrets policy between partners.
Both partners sign the same form.

Good Faith Estimate
2 pagesFor clients who are uninsured or paying out of pocket. Federal law (the No Surprises Act) gives you the right to a written estimate of expected costs. Review it and sign before your first session.
Release of Information — Insurance
2 pagesIf you're using insurance or seeking reimbursement, this allows the details your insurance company needs, like dates of service and billing codes, to be shared for claims.
Release of Information — Another Person
2 pagesIf you'd like Shelbie to talk with someone else about your care, such as another provider, a family member, a school, or an attorney.
Each form covers one person, so fill out a separate one for each.
